Self-Management Programmes for Long Term Conditions
Helping people take control of their health and their life
The Whittington Health Self-Management and Behaviour Change Service supports local people to be engaged in their own health and wellbeing as much as possible, offering programmes for people living with health conditions to develop their confidence, knowledge and skills to actively self-manage.
Courses will be held regularly throughout 2018 and will take place at the following locations:
- Tynemouth Road Health Centre
- Cypriot Community Centre, Wood Green
- Hornsey Central Neighbourhood Health Centre
- Lordship Lane Health Centre

Expert Patients Programme
The Expert Patients Programme (EPP) is a free 6-week course for people living with any long-term health conditions or their carers. The course is 2.5 hours per week and is delivered by trained lay tutors who themselves live with long-term conditions. EPP delivers practical evidence-based tools as well as drawing on tutors’ own experiences of successful self-management, helping participants to manage their health condition and feel more in control. Participants learn how to:
- Manage symptoms
- Cope with stress, anxiety and depression
- Get the most out of local services and relationships with health professionals
The course also covers problem-solving, decision-making, goal-setting, relaxation techniques, healthy eating and more.

Diabetes Self-Management Programme
The Diabetes Self-Management Programme (DSMP) is a free 7-week course for people who live with type 2 diabetes. DSMP helps participants understand more about diabetes and develop their knowledge, confidence and skills to manage their diabetes more effectively. Topics covered are similar to EPP with additional diabetes-specific components. The 3 hours per week is co-delivered by a lay tutor living with type 2 diabetes and a healthcare professional. Participants report feeling more confident to take control and make positive behaviour changes.
DSMP is suitable for those who have had diabetes for some time and may be struggling to manage, or for those who may be more recently diagnosed and have had some diabetes education (e.g. Conversation Maps) but would benefit from support in putting their learning into action.
